New Student Registration

This page is intended for students (grades K-8) who are NEW to Fairview School District 72 for the 2020-2021 school year.
Families moving into District 72 during the summer may register their child as part of a four-step process. All four steps are required before your child is fully registered with District 72.

Step One (online): Email Daniela Termini in the school office- Include the number of incoming students in your email. You will receive an email back with paperwork including a mandatory residency verification form.

Please note, residency is defined as having a permanent presence at a residence with evidence of living and sleeping in the home. Documentation of closing on home or proof of lease prior to enrolling in the district is needed. Upon completion of this step, your address will be verified that it is within the boundaries of the district. You will be contacted by the school once this step is complete with a link for registration Step Two (online): Complete the Online Registration Form which includes:

Demographic and Contact Information Emergency Medical Information Handbook Agreement and Permissions
Step Three (online or in person):  Forms
•      Copy of Birth Certificate Vision forms required for kindergarten students Dental forms are required for kindergarten, second grade, and sixth-grade students Emergency Medical forms are required for ALL students Concussion Form is required for ALL students (online) Athletic Permit is required for all students, grades 6-8, planning to participate in any sport during the school year.
